Our Skilled Physical Therapy Services Include:

• Thorough evaluation of movement and function
• Development of custom exercise programs that can be done at home for strength, endurance, coordination and balance. These programs can be focused on:
  • restoring movement and function, such as when recovering from a surgery or
  • preventing decline in function, such as when a debilitating disease is affecting someone's ability to manage their daily activities
• Training of how to walk and move safely
• Evaluation of safety in the home with specific recommendations for what changes can be made to prevent falls
• Breathing exercises and strategies to conserve energy for those with heart and breathing problems
• Teaching to caregivers so that they may assist in implementing the therapy plan

Our Skilled Medical Social Worker Services Include:

• Thorough evaluation of physical, financial and emotional challenges
• Teaching of strategies to manage stress and cope with challenges
• Assistance with applications to various community services, acting as an advocate when needed
• Counseling
• Assistance with applications for financial assistance
• Assistance with determining the best residential and care setting based on functional ability
• Assistance with evaluation of and access to alternative housing environments

Our Skilled Occupational Therapy Services Include:

• Thorough evaluation of how daily living activities such as bathing, dressing, preparing meals, and doing laundry are completed
• Thorough evaluation of how problems with movement, weakness, vision, breathing, nerve damage, mental function and other challenges affect getting daily activities completed
• Teaching designed to address any challenges and promote safety and independence in daily living activities
• Evaluation of and fitting for devices and equipment that make daily activities safer and easier to perform
• Teaching of how to make activities easier to conserve energy when heart or lung problems limit endurance
